NEW PATIENTS

The doctors welcome new patients who live within our practice area. Please check with reception to see if your address falls within our catchment area. You are asked to submit proof of address (Tenancy agreement, council tax bill, utility bill or bank statement in last 3 months ) , your NHS number and passport or photo ID. These documents are not a mandatory requirement and you will not be refused registration if you cannot produce them. If you have any difficulties in providing documents asked for , our friendly practice manager will ring and assist you to facilitate registration. Our aim is to make sure that the registration process is smooth and stress-free for the patient.

Whilst we can accept registration forms at any time in person (excluding Wednesday afternoon when we’re closed), it would be very helpful if you are able to attend on the following days/times as these are our least busy periods: Monday – Friday 11.00am-12.00 noon Monday, Tuesday, Thursday, Friday 2.00pm – 3.00pm (please note we are closed Wednesday pm)

As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a new patient health check with the nurse.

Please note- For children under 5 yrs , we will require the red book during registration medical.

Once an application is submitted , we aim to approve and make the appointment to see nurse for registration medical check up within 2 weeks. Please tell receptionist, if there is a need to see the doctor sooner.

Once you are fully registered with the practice , you will be automatically sent a letter from NHSE confirming registration with practice.

If we are unable to accept your application , we will write to you within 2 weeks giving you the reasons.

CHANGE OF ADDRESS & PHONE NUMBER

Please inform us immediately if you move, your phone number changes or if you change your name. Please submit copies of marriage certificate or deed poll at the reception to change your name. Please submit a signed and dated change of address form to change your address and/or phone number.

If you move out of the practice area , you are required to register with another doctor closer to your new home.

CAR PARKING

The surgery car park has limited parking . There is designated parking for disabled as well as for parents with toddlers. Off street parking is also available around the surgery

DISABLED ACCESS

Disabled access is via the front entrance. The clinical rooms are mainly accommodated on the ground floor of the building.

REPEAT PRESCRIPTIONS

Patients on regular medication may require a prescription. You will find attached to your previous prescription a tear-off slip with your medication listed. Please tick the medication you require and bring or send it to the surgery. Your prescription should be ready in 2 clear working days from receipt of the request. Scripts are sent electronically to nominated pharmacy.

Housing Applications-If you are intending to apply for a council home for health reasons you do not need a doctor’s letter prior to application. You may apply stating your circumstances and the Council will write, in confidence, to the GP, if further information is required

Police requests for reports on our patients about the suitability to hold firearms

The proper regulation of firearms is in the public interest.However, we are not contracted to provide reports on suitability to hold firearms under our terms of service. Furthermore, we do not provide a report because we have a conscientious objection to the holding of firearms.

SICKNESS CERTIFICATESYou do not need an NHS sick certificate until you have been off work for one week. You will usually need to see your doctor to obtain one after this.TEST RESULTSYou may not always need to see your doctor for the results of your tests. If you are asked to phone, please phone 01895 233881 after 12pm and explain to the receptionist the nature of the test and the result you need. Please note-We never give pregnancy and sexually transmitted diseases(STD) screening results over the phone. You have to see the nurse to discuss those results.

Your NHS Number, Keep it Safe.

Every person registered with the NHS in England and Wales has their own unique NHS Number. It is made up of 10 digits for example 123 456 7890.

Everyone needs to use the NHS Number to identify you correctly. It is an important step towards improving the safety of your healthcare.

Always bring your NHS number with you to all appointments or quote it if you need to telephone the practice for any enquires. This will allow staff to check that they have the right patient details by checking this against your NHS number.

To improve safety always check your NHS Number on correspondence the NHS sends to you.

Ways of finding out your NHS Number

If you do not know your NHS number, contact your GP or CCG. You may be asked for proof of identity, for example a passport or other form of identity this is to protect your privacy.

Patient Relations

We will do our best to help you to resolve any concerns you may have about the care you received.

If you have a concern or there is a problem, the best way to get it resolved is usually to tell someone there and then. In Surgery, talk to the Doctor, nurse or receptionist on duty. If you want to talk to a senior manager or to someone who has not been directly involved in your care and treatment, we can usually arrange this during office hours.If you wish to make a formal complaint you can telephone or write to: Mrs Yvonne Blowfield , Practice manager
